Karui Fortress is a static Layout, it will always be the same. Move East at the Start till you find the Entrance to Tukohama Boss Arena.
After the Fight exit through the North Entrance and move East again till you can move North to find the Exit to Ridge.
Karui Fortress is based on the Fetid Pool Layout from Part 1 Act 1. A Zone we do not enter during a optimized Act 10 Run.
Fetid Pool / Karui Fortress is a circular Zone around a Lake. Karui Fortress is static, so we can always run it the same Way.
The Exit to the Ridge is only unlocked, once you kill Tukohama, it is a forced Boss Fight.
